The novel is framed by an elderly Tricia's letter to Rainey, which revisits the social rituals and daily humiliations of life as a military dependent in 1963 Saigon. Tricia navigates markets, patronizing luncheons, and an overwhelming sense of inadequacy as she struggles to fit into Charlene's world. Her miscarriage is met with unexpected empathy from Charlene, who confesses her own loss, briefly shattering the image of perfect womanhood Tricia had projected onto her. Tricia's college friendship with the firebrand activist Stella runs in parallel, illuminating her lifelong tension between idealism and self-preservation. After withdrawing from Charlene's escalating work and eventually undergoing a hysterectomy, Tricia quietly rebuilds her life through teaching and accepts that motherhood will not be part of her story. Decades later, Rainey encounters an elderly Dom and his adopted son Jamie, and their shared recognition of a Saigon Barbie on his shelf leads to a sudden tragedy that finally prompts Rainey's letter to Tricia.

As their friendship deepens, cracks appear in Charlene's image of perfection: her dependency on alcohol and pills, her open infidelity at the leprosarium, and her willingness to push Tricia into danger. Revelations accumulate around the enigmatic American doctor, whose nature no two women can agree on, and around Peter's quiet paternalism, which culminates in his booking Tricia's return flight without her knowledge. In Rainey's letter, Dom fills in pieces of Charlene's story—including the moment she held a dying child while he could not—and reveals that his son Jamie was an abandoned hospital baby he and his wife chose to adopt. The story resolves with Tricia correcting her own earlier account: Charlene had arranged an illicit adoption that Tricia ultimately refused, and the novel closes on the quiet revelation that Lily chose to remain behind at the leprosarium for good.
