The novel follows Cassie Spencer, a 24-year-old rising country singer-songwriter, and Haden Westbrook, the 26-year-old top ranch hand at Silver Pines Ranch in Laurel Creek, Kentucky.

Cassie arrives at Silver Pines to meet her newborn niece, Billi, the daughter of her older sister Ivy and Ivy's husband Wade Ashby, who owns the ranch. Eager to record a social media video for her demanding manager, Dax O'Brien, Cassie persuades a young ranch hand to let her ride Outlaw, an unbroken quarter horse. Haden spots the danger, but Outlaw bucks Cassie to the ground. He catches her when she collapses, carries her to the barn, and tends to her injuries. They trade flirtatious insults, each insisting the other is not their type. Cassie learns his name, but Haden never learns hers.

That evening at the Horse and Barrel, the town's pub, they meet again. They play pool, banter, and dance, and their chemistry takes over. They have sex in the cab of Haden's pickup truck. He volunteers nightly at Penny Lane Rescue Ranch, run by Penny Garrison, a 68-year-old veterinarian who has become his surrogate mother. Cassie shares her own losses: her father's death when she was ten, her mother's alcohol addiction, and how Ivy raised her. She begins therapy with Dr. Payler and starts writing music again. She also bonds with Marlow, a neglected rescue horse she visits daily.

On a girls' night out with Olivia Sutton and Ginger Danforth, two women who become close friends, Cassie confesses her history with Haden. Ginger points out that "Your Truck" misrepresents what happened: Haden did not leave Cassie; she left him. One evening, Cassie follows Haden's truck and discovers Penny Lane. He introduces her to Penny, and Cassie helps with the rescue horses, moved by his gentleness and his dream of owning his own rehabilitation facility. On the drive home, she apologizes for ghosting him and for the song. Haden admits the lyrics hurt not because anyone else knew the song was about him, but because he did. They agree to be friends.

A heavy snowstorm knocks out power across the ranch. Haden brings Cassie to his cabin, and the evening escalates into a passionate encounter. Afterward, she breaks down crying about the woman who died at the concert. Haden holds her, telling her she is safe. The next morning, part of Penny Lane's barn has caved in under the snow. Volunteers rescue the horses, though one does not survive. At an Ashby family dinner, CeCe, the wife of Wade's brother Nash, proposes a benefit concert at Olympia, Nash's sports arena, to raise funds. Back on the road, Cassie uses her coping techniques to manage performances while staying in contact with Haden. Her producer rejects the last song on her album, so she plays him "Baby Be Mine," written at Silver Pines, and he accepts it immediately. She discovers that Dax has booked a 40-plus-city tour without her consent. With guidance from her lawyer, Cassie learns her management contract expired months ago and was never renewed. She fires Dax and hires Fiona Teller's firm as her music publisher. Cassie plays Wagon Wheel as her final live performance, then announces she is stepping back from the stage to focus on songwriting.

Meanwhile, Haden purchases Penny Lane from Penny, fulfilling his lifelong dream. He confronts his father, Jim, who shows up uninvited and insults his choices, telling Jim he will no longer tolerate being belittled.

Cassie flies to Kentucky in secret for the benefit concert. Evan Woods introduces her as a surprise guest, and she performs a new song, "Kentucky and You," declaring she has found her home in Kentucky and in Haden. He weeps openly in the audience. Backstage, Cassie tells him she has quit performing and wants to write music from his front porch.

They move into the Penny Lane farmhouse together. Before the wedding of Wade's brother Cole and Ginger, Haden's father visits to deliver an emotional apology, admitting he projected his failures onto his son. Haden agrees to a fresh start, telling Jim that family does not run out of chances.

An epilogue set seven years later opens on the morning of Cassie and Haden's wedding. They have a four-year-old daughter, Quinn, and a nine-week-old son, Beau. Penny Lane has expanded to three barn sites, and Cassie has won a Grammy for songwriting. Their ceremony is a simple affair under an oak tree on their property, with Quinn walking down the aisle holding Cassie's hand. Haden reflects that Cassie is the first person who left and came back, affirming that what is meant to be will find its way.
