The novel opens on a snowy Oregon morning as the Hall family gathers for breakfast before a catastrophic car accident kills Mia's parents and leaves her critically injured in a coma. Mia's consciousness escapes her body, allowing her to observe the hospital chaos as her grandparents, best friend Kim, and boyfriend Adam arrive to support her. A series of escalating crises — denied hospital access, a confirmed family death, and a Code Blue emergency — test whether she has the will to survive. Adam's dramatic efforts to reach Mia's bedside, culminating in Willow's intervention, bring the person Mia most needs to her side. In her darkest hour, Mia's grandfather offers her unconditional permission — to stay or to go — which eases her immense burden. The novel concludes with Adam playing Yo-Yo Ma's cello music for Mia, and her response — squeezing his hand — signals her choice to live.

Long before the accident, Mia's identity was forged by her deep connection to the cello, from her first enchanted encounter with the instrument to her early recitals and Professor Christie's mentorship. Her best friend Kim played a crucial role in keeping her on track, convincing her not to quit the cello and urging her to attend a prestigious summer conservatory where she blossomed among fellow prodigies. Gran's suggestion that Mia audition for Juilliard set the arc of her ambitions in motion, a dream supported by family who drove her hundreds of miles to make it possible. At the hospital, those same relationships sustain her — Adam and Kim bond while scheming to reach her, Gran offers tender comfort, and Kim reminds Mia of all the love she still has in the world. These secondary moments form the emotional bedrock underlying Mia's eventual choice to survive. Adam's final plea at her bedside — promising to release her to her dreams if she simply chooses to live — is the culmination of all this accumulated love and sacrifice.

The story's most pivotal turning points begin even before the crash, with Beethoven on the radio in the moments before impact and Mia's consciousness escaping her body to confront the devastating truth of her situation. Early hospital revelations — that she has barely minutes to live and that her fate rests entirely in her own hands — establish the existential stakes driving the entire narrative. Woven through her coma, key flashback decisions define her relationships: Adam's declaration of admiration on their first date, their intimate musical breakthrough in her bedroom, and the high-stakes question of whether Mia will leave for Juilliard. Her mother's kitchen-sink wisdom and her father's quiet philosophy about choice prepare Mia emotionally for the impossible decision she now faces. As Mia edges toward choosing death while humming her late father's lyrics, the weight of every past turning point converges on a single moment. The final image of Mia playing cello alongside her father and Adam at a joyful barbecue represents precisely what she stands to lose — and exactly what makes choosing life worthwhile.
