Alaina Smith makes a full recovery; shortly after, another child with balamuthia dies because her doctors learn of nitroxoline too late.

Michael Lewis identifies a journalistic goldmine inside federal agencies after witnessing a presidential transition that left key departments without knowledgeable leadership. Mining engineer Christopher Mark spends a career developing a safety formula that eventually eliminates roof-fall fatalities in US coal mines entirely. Ronald Walters transforms the National Cemetery Administration into the highest-rated organization in the country by treating every veteran's burial with equal dignity. Scientists at NASA's Jet Propulsion Laboratory and IRS cybercrime investigators pursue missions that only government funding and legal authority make possible. Pamela Wright opens the National Archives to ordinary citizens through digital innovation, while young Olivia Rynberg-Going begins her career enforcing antitrust law in Washington. The collection ends with a life-or-death race against a rare brain-eating amoeba, in which an FDA employee's quiet work helps save a child's life even as the information gap that could prevent future tragedies remains dangerously unfixed.

The collection grows from Lewis's earlier book The Fifth Risk, which he compiled after exploring agencies whose work went unrecognized during a turbulent political transition. Christopher Mark's path to mine safety research winds through youthful radicalism, personal tragedy, and an academic pivot that transformed his sense of purpose. Ronald Walters builds on his advanced education to create programs that honor veterans in death and support them in life, including career training for unhoused veterans. IRS investigators crack major cybercrime cases by studying the very technologies criminals exploit, while NASA astronomer Vanessa Bailey adds to humanity's tiny catalog of directly imaged exoplanets. Writer Sarah Vowell uncovers census records using AI-assisted release, and W. Kamau Bell traces Olivia Rynberg-Going's politically shaped childhood before following her to Washington. The final essay pivots on researcher Joe DeRisi's discovery that a single drug can defeat a deadly amoeba—knowledge that exists but struggles to reach the doctors who need it most.

The personal journeys and behind-the-scenes milestones that shaped each essay's story.

Lewis's central revelation—that government agencies contain extraordinary people solving critical problems in total obscurity—frames every essay that follows. Christopher Mark's career turns twice: first when he abandons activism for science, and again when he discovers that a widely adopted safety tool had never actually been used to improve safety. At JPL, Nancy Grace Roman's long-held vision of a planet-hunting telescope and Nick Siegler's midlife career change both illustrate how personal commitment shapes the future of public science. Lanchester exposes a troubling disconnect between measurable economic reality and public feeling, while IRS investigators secretly reverse-engineer terrorist fundraising through the same tools criminals rely on. Sarah Vowell's visit to the National Archives turns personally revelatory when she confronts the treaty that dispossessed her own ancestors, and Olivia Rynberg-Going's childhood is punctuated by the rise and fall of marriage equality. The collection's most painful turn comes when Alaina Smith's mother discovers a life-saving treatment through a simple online search—information her daughter's entire medical team had never encountered.

The discoveries, decisions, and emotional shifts that change the course of each story.
