This novel in the Orphan X series follows Evan Smoak, a former government assassin raised from age twelve in a covert initiative called the Orphan Program. Trained under his mentor, Jack Johns, Evan deserted the program and reinvented himself as the Nowhere Man, helping people who are terrorized and have nowhere else to turn. He operates from a penthouse in the Castle Heights Residential Tower in Los Angeles.

The novel opens with Evan intervening in an East Los Angeles attack. Five members of the Trinitarios gang pursue Lesandro Candella, a fifteen-year-old boy they mistakenly targeted, who has already lost his left forearm and a finger. Evan neutralizes all five with hand-to-hand combat, administers emergency medical care, and vanishes before police arrive.

Meanwhile, Anca Dumitrescu, a twenty-five-year-old Romanian-American woman who directs social services at a Romanian Orthodox church in the Bronx, rides the subway home past midnight. Anca has epilepsy and carries a laminated seizure plan around her neck. When she feels a seizure approaching, a young woman named Monica—who is en route to a rendezvous at Luke Devine's Hamptons estate—agrees to watch over her but departs at the 125th Street stop. As Anca loses consciousness, a group of young men enters her car and carries her off.

Separately, Luke Devine, an erratic billionaire who leverages surveillance footage from his Hamptons parties to blackmail politicians and world leaders, spirals through a manic episode. Monica, who witnessed Anca's abduction, is at Devine's estate but too shaken to explain what she saw. Unable to manage his state, Devine calls Evan's Nowhere Man line for the first time.

Evan is mourning Tommy Stojack, his closest friend and armorer who recently died. He flies to the Hamptons with Joey (Josephine Morales), his seventeen-year-old protégée and exceptionally talented hacker, who has been lobbying for a larger role in his missions. At Devine's estate, Tartarus, Evan uses a calculated strategy of confrontation and withdrawal to force Devine to surrender control of his operations, including handing over his black box—a small communication device he uses to broadcast commands to his staff. He installs Joey and Devine's chief of staff, Will Rawlings, as co-managers and gains access to Devine's intelligence apparatus, the Brain: an AI system with access to virtually every database, surveillance feed, and encrypted network worldwide.

From Monica's account, Joey tracks surveillance footage and identifies the abducted woman as Anca. Evan follows the trail to a squalid basement apartment in Harlem, where he finds a torn scrap of Anca's dress.

Anca regains consciousness in that apartment, her body battered and drugged, a slur scrawled across her cheek in permanent marker. She remembers ski masks, needle punctures, and hands on her body. Through a slit of vision she identifies a goat-skull tattoo on one attacker's chest. She walks over four miles to the Bronx because no one will help her, and Evan catches her when she collapses at her building.

Through a locked door, Evan coaxes Anca to preserve forensic evidence and agree to a hospital visit. Her examination reveals significant injuries, fentanyl in her system, and bruises consistent with gang rape. Anca chooses to ride the subway home, an act of courage to reclaim the space where she was taken. She insists her attackers will answer before God and refuses to authorize killing, telling Evan his desire for vengeance "is not yours to have."

Evan arranges for Candy McClure, also known as Orphan V, the deadliest female graduate of the Orphan Program and a woman with her own history of sexual abuse in foster care, to serve as Anca's bodyguard. Candy forms an immediate bond with Anca rooted in their parallel experiences of violation.

The investigation accelerates. Evan traces the apartment rental through a payment app to Manny Llorente, a pornography middleman who edits and distributes assault footage for a group called the "White On Posse," four young men who record their attacks. Evan discovers a flash drive of Anca's assault, neutralizes Manny, and contacts federal authorities. He also establishes a legal channel with Deputy Assistant Director Naomi Templeton of the Secret Service, a longtime adversary, proposing a partnership: He will drag criminals into the light, and she will prosecute them. He separately extracts Anca's promise that she will not kill her attackers.

Evan watches the full recording of Anca's assault for forensic identification, an experience that overwhelms his defenses and leaves him weeping. He tasks Joey with erasing every trace of the video from the internet. The case distorts his judgment in a parallel mission: When a girl calls his line because an older boy is extorting her with a nude photo, Evan responds with disproportionate violence, recognizing afterward that he used the encounter as an outlet for rage.

The narrative introduces the White On Posse through Taz (Taswell Kinley), their nineteen-year-old de facto leader, whose attention is fractured across screens and substances. His crew includes B-Roll (Brandon Burke), who lures victims through charm; Finn-Finn (Finley Jacowski), a dim follower; and Big Mikey (Michael Macmanus), whose cousin Dirty Pete is a violent ex-convict. They begin grooming a high-school girl named Blanca for their next assault.

Evan infiltrates RedLite, the pornography platform hosting the group's content, posing as a blogger. He plants malware-loaded flash drives throughout the office; once an employee plugs one in, Joey's self-replicating worm devastates their databases. At a speakeasy dinner afterward, Evan lashes out at Joey, masking his fear for her safety with operational coldness. A phone call with Aragón Urrea, a billionaire ally who has placed his private jets at Evan's disposal in gratitude for a past mission, helps him recognize that his overprotectiveness reflects his own blind spots.

The White On Posse hires Dirty Pete to eliminate Evan. After Anca gives her formal statement to federal prosecutors, she and Evan are ambushed at Glass Bottle Beach in Brooklyn. Pete's twelve-year-old son, posing as an injured cyclist, injects Evan with fentanyl, giving him roughly four minutes before unconsciousness. Evan fights three adult attackers as the drug overtakes his senses, neutralizing all of them before collapsing in Anca's lap. She guards him while he is unconscious and kicks Pete unconscious when he crawls toward them. Evan wakes with a tension pneumothorax, a condition in which trapped air threatens to collapse his lung, and improvises a decompression at a gas station using a ball-pump needle and vodka.

Joey identifies two rapists through biometric analysis, and Evan confronts Finn-Finn and Big Mikey at a diner, extracting the remaining names. He races to B-Roll's apartment, where B-Roll has lured Blanca and prepared cameras for a live-streamed assault. Evan arrives before the assault begins, sends Blanca safely out, and locks B-Roll into his own pillory before texting Templeton the location.

With all four members arrested, charged, and held without bail, Evan says goodbye to Anca outside her church. She tells him the men are now in better hands than the law, meaning God's. In a conversation with Mia Hall, the district attorney who lives in his building and who has deduced he is the Nowhere Man, Evan begins to accept the cost of caring for others. Mia tells him raising someone requires a willingness to "surrender them to the world." In a final meeting with Joey, Evan confesses that watching Anca's video terrified him and articulates a new commandment: "If you feel more, you have to feel more."

Evan returns to Tartarus, where a recovered Devine outlines a plan to destroy RedLite financially and funnel settlement money to exploitation victims. Evan asks Devine to arrange a customized prosthetic arm for Lesandro. Devine mixes him a perfect vodka martini; Evan introduces himself by first name for the first time, returns the black box, and leaves. On Aragón's private jet heading home on Valentine's Day, Evan stretches with each limb toward a cardinal point, forming "a perfect X."
