The first book in the Asylum series follows Dan Crawford, a high school student and former foster child adopted by Paul and Sandy, as he arrives at New Hampshire College Prep (NHCP), a summer program for gifted students. Because the regular dormitories are under renovation, students are housed in Brookline, a former psychiatric asylum purchased by the college in 1972 and converted into a dorm. Despite fresh paint and landscaping, the building unsettles Dan. His unease deepens when he finds a faded photograph in his desk drawer of a bespectacled man in a doctor's coat whose eyes have been scratched out.

Dan's roommate, Felix Sheridan, a socially awkward biology enthusiast, mentions he explored a supposedly off-limits office that once belonged to the asylum's warden. At the welcome social, Dan meets Abby Valdez, a confident art student from New York, and Jordan, a math prodigy from Virginia secretly attending NHCP instead of a conversion therapy camp his parents believe he is at. The trio sneaks into the warden's office after hours and finds photographs of patients undergoing brutal treatments. Abby fixates on a framed photograph of a young girl with a jagged scar across her forehead and hollow eyes, signs Dan recognizes as evidence of a lobotomy.

That night, Dan opens his door and hallucinates a stone cell with an operating table and blood-stained shackles. Unable to sleep, he researches Brookline and learns about Dennis Heimline, known as the Sculptor, a serial killer who murdered over a dozen people between 1960 and 1965 by posing his victims like statues. When the asylum closed, no trace of the Sculptor was found. Dan begins having nightmares in which a faceless doctor approaches him with surgical instruments.

Over the following days, the three friends become inseparable. Dan asks Abby to dinner, and during the date he notices phantom emails in his Sent folder referencing "patient 361" that vanish before he can read them. The trio returns to the old wing and discovers a hidden passage behind a filing cabinet leading to deeper basement levels. Dan and Abby descend to a row of cells; Dan recognizes one as the room from his hallucination. They find a pristine inner office with the warden's personal effects. When Dan tries on the warden's spectacles, they fit perfectly, and his reflection reveals a disturbing resemblance to the man. Jordan finds filing cabinets of patient index cards, all marked "Homicidal: Y." Dan locates Dennis Heimline's card just before Joe McMullan, a hall monitor, catches them. Dan secretly pockets the card and a leather folder.

The Heimline card reveals the Sculptor was patient 361 and, uniquely, is marked "Recovered," meaning the warden apparently cured a serial killer. The leather folder contains a journal page describing the warden's obsessive quest to extract the "twisted root" of madness from patients' brains. Dan visits Sal Weathers, a local ghost hunter who runs a website about Brookline, but when Dan gives his name, Sal's wife screams and Sal throws him out. Back at the dorm, Dan finds a threatening note asking, "How do you kill a hydra?" and answering, "You strike at its heart." A professor had nicknamed the trio "the hydra" in class, making the note a direct, personal threat.

Abby reveals she found an index card for "Valdez, Lucy Abigail," her father's sister, admitted to Brookline in 1968 and never recovered. Abby's acceptance to NHCP feels like a sign to investigate her aunt's fate. Jordan reacts with hostility, and the argument fractures the group. Jordan confesses he has been having nightmares about being strapped down and shocked by faceless doctors, experiences that mirror the electroshock therapy once used to attempt to change people's sexual orientation.

Dan ventures into the basement alone and discovers a music box inscribed to Lucy in one of the cells. Deeper still, he finds an enormous underground operating amphitheater. He blacks out and experiences a vision in which he is the warden, preparing to operate on patients while authorities close in overhead. Abby finds him unconscious and revives him, and they kiss for the first time. As they emerge, police are swarming Brookline: Joe McMullan has been found dead, his body posed like a sculpture. Dan finds papers in his pocket that he does not remember taking, including a journal entry signed "Daniel Crawford, Warden," revealing the warden shares his exact name.

Dan calls his parents, who mention his history of mild dissociative disorder, characterized by memory lapses, and urge him to come home. Soon after, Jordan's roommate Yi is found unconscious and posed in the stairwell, and Dan's phone begins sending texts he has no memory of writing. When Felix is attacked on the fifth floor, police find incriminating messages in Dan's phone and assign him a police escort. Felix wakes in the hospital and clears Dan.

Dan discovers his notebooks contain passages in the warden's handwriting, identical to the threatening notes he has received. He realizes he has been writing the notes to himself during dissociative blackouts. At the Camford Baptist church, pastor Ted Bittle confirms the warden was the eldest of three Crawford brothers and that the youngest had a son around 1972, meaning that child could be Dan's biological father and the warden his great-uncle. The pastor reveals the warden was killed by another inmate before his trial concluded and that the Sculptor's body was never found.

Dan discovers Sal Weathers's body in the woods, posed like a sculpture. Back at the dorm, a note in unfamiliar handwriting appears under Abby's door: "It's time for treatment. Come to the basement at midnight" (276). Dan and Abby descend while Jordan enters separately to call police. Someone shoves the filing cabinet over the passage, trapping Dan and Abby, and a figure knocks them unconscious.

They wake strapped to operating tables in the amphitheater, with lobotomy instruments beside Abby. Felix emerges in a white doctor's coat, speaking in a voice no longer his own. Channeling the spirit of the Sculptor, Felix confesses to killing Joe, attacking Yi, murdering Sal, and using Dan's phone to frame Dan. He declares Dan his "magnum opus," revenge against the warden who experimented on the Sculptor decades ago. Jordan cuts the lights and frees Dan. Felix charges with a scalpel. After a violent struggle, Dan overpowers Felix and straps him to the operating table. Dan picks up the scalpel and feels the warden's consciousness urging him toward violence, but he fights it, drops the blade, and thinks: "I'm not you. I will never be you" (297). Recognizing Brookline itself as the source of the malevolent influence, Dan insists they carry Felix out.

Police meet them on the stairs. Outside, Sal's wife attacks Dan before officers restrain her. Abby notices the scar on the woman's forehead, identical to the girl in the photograph, and realizes she is Lucy Valdez, Abby's long-lost aunt. Abby produces a chipped porcelain ballerina that she took from Lucy's music box in the basement, and Lucy recognizes it. Felix is hospitalized for psychiatric care, and Dan invites Jordan to stay with his family if things become unbearable at home.

The program shuts down. Professor Reyes, Dan's History of Psychiatry instructor, visits his room with a large set of skeleton keys, mentioning the old wing is locked permanently. Dan realizes she could have unlocked the wing from the start, raising questions about her role in events. As Dan and Abby step into the hallway, Dan finds a final note in the warden's handwriting: "I'll be seeing you real soon. Daniel Crawford" (310), indicating the warden's influence persists.
