Plot Summary

Dr. Laurel Harlow, a chemical dependency therapist in recovery from an alcohol addiction, is jolted awake near two in the morning by a frantic call from Gia, the director of Comeback Kids, a live-streamed reality TV show following seven former child stars through 28 days of inpatient addiction treatment at Crystal Meadows, a treatment center housed in a sprawling ranch in a gated Los Angeles neighborhood. Gia reports that Maddie Hernandez, the youngest client at 19 and the show's biggest draw, has bled out in the hallway. Spencer, another client with a history of domestic violence charges, has fled the house, and police have locked the property down as a crime scene.

Laurel races to Crystal Meadows and finds yellow police tape around the property, social media posts already asking whether Maddie is dead, and a coroner en route. When Gia emerges from the house flanked by police, she confirms Maddie is dead and persuades the officers to let Laurel inside. The house reeks of blood. A dark puddle seeps from Maddie's room, and a body bag sits near the back door. A tall female detective, later identified as Detective Boone, confronts Laurel and orders her removed, declaring Laurel cannot assist in a crime she may have been involved in.

Interspersed throughout the novel are third-person interludes titled "Her (Then)," which trace the life of an unnamed young woman whose inner light was extinguished on one traumatic night. She discovers self-harm by accident and escalates to dangerous extremes. Later interludes reveal she was gang-raped at a college fraternity party, reported the assault to police, and was relentlessly cyberbullied by anonymous accounts, sorority members, and strangers. These chapters are eventually revealed to chronicle the story of Paris Mathews, Gia's sister.

Laurel returns home at dawn and processes the night with her wife, Noelle, a kindergarten teacher. In their longstanding ritual, Noelle draws Laurel a bath while Laurel describes the surveillance footage: Maddie stumbled from her bathroom gripping her bleeding head, collapsed, convulsed, and cracked her skull against the wall. Javon, another client, performed CPR, but Maddie was pronounced dead on arrival. Laurel is consumed with guilt, having promised Maddie's mother, Hilda, that her daughter was in the best possible hands.

At the police station, Detectives Wallace and Boone question Laurel about the show's structure and her non-traditional treatment approach. They produce Maddie's tenth-step journal, a daily reflection notebook reviewed by staff and Laurel. Wallace reads alarming entries in which Maddie writes that she is scared, that nobody will believe her, and pleads for someone to look at the cameras. The final entry reads: "If something bad happens to me tonight, it wasn't an accident. Please tell my mom. I never wanted to die" (50). Laurel insists she never saw these entries and is certain the night staff never flagged them.

Laurel visits Gia and shares what the detectives revealed. They agree to split the investigation: Gia will track down Ethan, the night staff member, while Laurel searches for Spencer. Laurel locates Spencer at a park, high and paranoid. He mentions a notebook unprompted, saying "she" told him to deliver it and referencing Maddie's mother. His account is fragmented and cryptic, suggesting he knows something damaging about Laurel.

Laurel gathers the remaining clients at the Langham hotel for a group session, where several express an unexpected breakthrough: for the first time, they want to be alive. Before they can begin writing goodbye letters to drugs and alcohol, police officers arrive and take Laurel back to the station. Boone and Wallace confront her more aggressively, revealing that Spencer gave the notebook to an officer on his way out of Crystal Meadows the night Maddie died. They present Maddie's phone, which contains hundreds of intimate texts and calls with a contact saved as "My Love," traced to Laurel's phone number and linked to her Apple ID and Verizon account. Stunned, Laurel denies everything and leaves, telling the detectives to contact her lawyer.

Laurel visits Hilda, who reveals the cause of death: anaphylactic shock from a severe allergy to ibuprofen. Hilda also discloses that Gia gave her a phone on Maddie's first day, claiming it was part of a research study on whether contact with loved ones aided recovery. Laurel is shaken, remembering that Gia handed her pills that afternoon for Maddie's headache. If those pills were ibuprofen, Gia orchestrated Maddie's death and used Laurel as the unwitting delivery mechanism.

Laurel returns to Crystal Meadows at night and reviews surveillance footage. She finds video of Gia handing Spencer the journal under an AC vent that blocks audio, and footage of Gia passing her pills in a brief hallway exchange, followed by clear video of Laurel giving those pills to Maddie. She records everything on her phone and rushes home to check her Verizon records. Instead of proving the texts were faked, she discovers Maddie's number on their family plan, with hundreds of calls and texts made late at night while Laurel slept. The only person with access to her phone at those hours is Noelle.

When Laurel confronts Noelle, Noelle feigns confusion, then urges Laurel to sleep and call a lawyer. When Laurel insists on going to the police, Noelle leaves the room and returns with a baseball bat, striking Laurel in the forehead and knocking her unconscious.

Laurel wakes restrained in Gia's soundproof guesthouse, her nose broken and her right eye swollen shut. Gia and Noelle enter together, and Laurel realizes they are romantically involved. Gia reveals her real identity: Georgia Mathews, whose sister Paris was a pledge at Alpha Kappa Alpha at the University of Southern Mississippi, Laurel's alma mater and sorority. Gia tells Laurel that she left Paris alone in a back room at a party where Paris was gang-raped by fraternity members. Paris was subsequently bullied, and the final text she received before dying said "Kill yourself, bitch" (169), sent from Laurel's phone. Laurel reveals to Noelle that she too was raped during pledge week and argues she was a traumatized teenager with no memory of sending any message to Paris. Noelle is unmoved.

When Noelle briefly unties one of Laurel's hands, Laurel smashes Noelle's face into the bed frame and runs outside, but Gia and Noelle overpower her at the fence and drag her back inside. A late "Her (Then)" interlude reveals the full truth: Paris did not die by suicide. After deciding to keep living following a meeting with a district attorney, Paris was ambushed in her apartment by Aaron, her rapist, and his friends. They drugged her and staged her death using a pre-written goodbye letter, then had the sorority president arrange for Laurel to send the final text. Laurel was manipulated into sending the message without understanding its purpose; Paris was murdered.

As Laurel grows increasingly ill from what she suspects is poison mixed into the soup Gia feeds her, Gia forces Laurel to write a false confession admitting to an affair with Maddie, to knowingly giving her ibuprofen, and to her role in Paris's death, threatening to torture Noelle if Laurel refuses. Laurel complies. Gia then feeds Laurel a popsicle laced with opiates, rendering her paralyzed. They bathe Laurel at her own home and, on Gia's countdown, each slice one of Laurel's wrists with razor blades. As the bathtub fills with blood, Laurel experiences a peaceful death, seeing visions of her deceased father and grandmother. She forgives Noelle and lets go.

The epilogue shifts to Noelle's perspective three days later. She waits at a trailhead for Gia, following their agreed cover story: Noelle was at the Four Seasons when Laurel died by suicide. Gia never appears. Her burner phone is disconnected. When Noelle calls Gia's regular phone, an assistant reveals that Gia has flown to Germany to be with her husband, a reclusive photographer Noelle never knew existed. The assistant mentions that police believe Laurel killed Maddie. Noelle realizes Gia was supposed to call police for a wellness check so they would find Laurel's body and rule the death a suicide, but Gia never made the call. She has fled the country, leaving Noelle alone with all the evidence. Noelle drives home, opens the door to the smell of Laurel's decomposing body still in the bathtub, and collapses.
