Plot Summary

Georgia Cartwright, a 32-year-old high-end wedding planner from a wealthy Charlotte, North Carolina, family, wakes up restrained in a locked psychiatric ward. She has been committed after police found her standing over the body of her younger sister, Annabelle, who was bludgeoned to death with a heavy sterling-silver paperweight at the family estate. Georgia appeared to be in a dissociative state, a condition of detachment from one's thoughts, feelings, and surroundings. Alden, the police chief's son and Georgia's ex-boyfriend, ordered a psychiatric hold rather than arresting her. Georgia's mother, Honey Cartwright, has publicly disowned her, and the media has all but convicted her.

Ninety minutes away, Mandy Ravenel, a 32-year-old bar owner, receives a call from Georgia's public defender, Milt Daniels, who reveals that Georgia is Mandy's twin sister, separated from her at birth. Mandy, who is adopted, is stunned but feels an instinctive pull toward Georgia and drives to the hospital.

Inside the ward, Georgia appears dull and vacant, but when she and Mandy are close enough to whisper, her eyes sharpen with full awareness. She insists on her innocence and begs Mandy to help free her. She pulls out strands of her own hair for DNA testing. Inside the ward, a volatile patient named Josh fixates on Georgia, stalking her through hallways and cornering her in the shower. A nurse's aide named Opal discovers Georgia's act and threatens to force-medicate her. Georgia's only apparent ally is Patty, a middle-aged woman admitted after a suicide attempt, who intervenes when Josh threatens Georgia.

Mandy investigates aggressively. She attends Annabelle's funeral, infiltrates the reception at the Cartwright estate, and photographs the guest book. Georgia arranges for Mandy to collect her personal belongings, which include apartment keys. Inside Georgia's penthouse, Mandy discovers eerie parallels between their lives: similar furniture, the same groceries, and the same novel on their nightstands. She also finds $10,000 in cash, which she uses to fund her investigation.

Through flashbacks, Georgia reveals how she learned of Mandy's existence. She hired a private investigator named Tony Wagner, who found that Georgia's real birth certificate contained a box marked "Twin" that was blank on the version she had always possessed. Tony located Mandy and sent someone into her bar with a hidden camera. Georgia had known about Mandy before Annabelle's death but had not yet reached out.

Georgia also reveals the secret she believes will prove her innocence. As children, she and Colby Dawson, the senator's son, bonded as outsiders in their powerful families. Colby told Georgia his father sabotaged his attempt to date Annabelle, and Georgia witnessed the senator in an intimate moment with Annabelle at a Christmas Eve gathering. When Colby reported that the senator abruptly left a family dinner on the night of his wedding anniversary after receiving a text from Annabelle, Georgia hired Tony to surveil Annabelle's apartment. Tony captured video of the senator leaving the building late that night.

Mandy finds the video on a thumb drive in Georgia's mail but is furious to discover additional surveillance footage of herself, revealing Georgia had been watching her. She visits Tony's office but finds him dead at his desk, killed less than an hour after their phone call. She secures the thumb drive in a bank safe-deposit box.

Mandy also uncovers her own family's secrets. Her father's tax returns show his income quadrupled the year she was born, paid by a Charlotte company. A St. Michael the Archangel figurine in Georgia's closet matches one Mandy's parents gave her at birth, proving her parents knew about Georgia. A Charlotte bartender recognizes Mandy's father from a photograph and says he once worked for the Cartwrights before vanishing amid rumors of theft.

The situation escalates inside the ward. Kyle Dawson, the senator's ruthless older son, visits Georgia and delivers an ultimatum: plead guilty and accept 20 years in prison, or be killed. Then Patty, Georgia's seeming ally, returns after her discharge and drops her disguise. She is a covert operative for the senator who deliberately injured herself to gain admission and monitor Georgia. Patty reveals that Georgia's fingerprints have been placed on the murder weapon and orders her to plead guilty.

Georgia reflects on how Honey systematically distorted the stories that branded her as cruel toward Annabelle. In the stroller incident that defined family lore, Georgia was trying to save baby Annabelle when a faulty brake released, but Honey told everyone Georgia pushed the stroller into traffic. After years of estrangement, Annabelle witnessed a similar malfunction, questioned the family narrative, and called Georgia. The sisters reconciled, and Georgia told Annabelle about Mandy.

Mandy texts Honey that she has information about Annabelle and the senator, then goes to the Cartwright estate. In the windowless library, she reveals her true identity as Georgia's twin. Honey, shaken, admits Mandy was never supposed to return. She explains that Mandy's father, Ray, was the family butler. When Honey's twin pregnancy produced two babies and she was already expecting Annabelle from her affair with the senator, she paid Ray and his wife to take one baby and leave Charlotte. The theft rumor was Honey's invention.

When Mandy mentions that DNA testing would expose Senator Dawson as Annabelle's biological father, Honey pulls a gun from a hollowed-out book. Mandy notices Honey is left-handed, meaning a left-handed strike would land on the right side of a victim's head, exactly where Annabelle's wound was. Mandy shouts that Honey killed Annabelle. Honey fires, and the bullet slams into Mandy's midsection. Senator Dawson, who had been upstairs, bursts in, disarms Honey, and restrains her. Honey confesses: Annabelle obtained DNA results proving the senator was her father and wanted to reveal the truth publicly, so Honey struck Annabelle in a panic and framed Georgia.

After the senator gives his statement to police, Georgia is released from the ward. She visits Annabelle's grave and encounters her father, Stephen Cartwright, who reveals he always knew Annabelle was not his biological daughter and loved her regardless. He promises to be present for Georgia going forward. At the hospital, Georgia finds Mandy recovering. The bullet never pierced Mandy's body: Before visiting the Cartwrights', Mandy tucked the St. Michael figurine from Georgia's closet into her bag. The bullet lodged in the solid metal, and Mandy's ribs broke only from the impact. Georgia clarifies that the senator's late-night visit to Annabelle's apartment was not romantic; Annabelle had called him for help after an arrest for driving under the influence. The sisters agree to destroy the thumb drive.

In the epilogue, Mandy retrieves the thumb drive from her safe-deposit box. She also handles a sealed envelope of Polaroid photographs she has kept there for over a decade. The photos document what Mandy did to Bradley, the man who raped her college roommate, Beth. Mandy tracked Bradley, drugged him, photographed him in humiliating poses, then pushed his unconscious body into a campus fountain. He did not wake up, and she watched him drown without intervening. No one ever discovered what she did. As Mandy leaves the bank, she reflects that the public suspects the wrong Cartwright sister of being a killer: They have the right idea but the wrong twin.
