Levitin opens with a historical frame. Around 5,000 years ago, the invention of writing allowed merchants to externalize memory for business transactions. He argues that human memory fails not because of storage limitations but because of retrieval limitations: Memories can be incomplete, distorted, or overridden by vivid anecdotes that overwhelm more reliable statistical evidence. The brain retrieves memories through associative access, the ability to reach a memory through multiple cues. External memory systems, from clay tablets to smartphones, either replicate the brain's organizational schemes or reinvent them to overcome its constraints.

The modern challenge, Levitin contends, is severe. Americans took in five times as much information daily in 2011 as they did in 1986, and tasks once handled by specialists have been off-loaded onto individuals through what he calls shadow work. The conscious mind can process only about 120 bits per second, meaning a person can barely follow two simultaneous conversations. This mismatch creates decision overload: The brain's decision-making network does not distinguish between trivial and important choices, so selecting a pen depletes the same neural resources as a consequential financial decision. Levitin identifies conscientiousness, comprising industriousness, self-control, and a desire for order, as the best predictor of positive life outcomes.

Levitin describes four components of human attention: the mind-wandering mode (also called the default mode network), a resting brain state discovered by neuroscientist Marcus Raichle that generates loosely connected, creative thinking; the central executive mode, responsible for focused cognition; the attentional filter, which selects stimuli based on change and importance; and the attentional switch, located in a brain structure called the insula, which directs neural resources among the other modes. The mind-wandering and central executive modes operate in opposition: When one is active, the other is suppressed. This architecture explains why losing keys is common. Parts of the brain can fall asleep momentarily, and if the attentional system is occupied when someone sets something down, no memory of the location is encoded.

Memory, Levitin explains, is reconstruction rather than faithful replay. Each experience activates a unique neural network, and remembering involves reactivating it, but the instructions degrade over time. Two principles govern which memories persist: distinctiveness (unique events face no competition during retrieval) and emotional significance (neurochemical tags mark important events for easier access). He demonstrates memory's fragility by citing psychologist Elizabeth Loftus's research showing that a single word change in a question can double the rate of false recall.

Categorization is one of the brain's most fundamental operations. Drawing on cognitive psychologist Eleanor Rosch's work, Levitin explains that the brain defaults to a basic level of description, treating objects within a category as functionally equivalent to conserve resources. Neuroimaging confirms that categories occupy specific biological locations in the brain. As a practical tool, Levitin presents the 3 × 5 index card system: Writing one idea per card enables random access and easy sorting. He cites efficiency expert David Allen's method of externalizing the rehearsal loop, a frontal cortex-hippocampus circuit that keeps unattended tasks cycling in consciousness, by writing down every unfinished thought and sorting tasks into four categories: Do it, Delegate it, Defer it, or Drop it.

Applying these principles to homes, Levitin notes that the hippocampus evolved to track stationary objects, which explains why people remember where their toothbrush is but not their reading glasses. The remedy is the rule of the designated place, supported by affordances (physical design features that prompt correct use) such as key hooks and phone trays. He warns against multitasking, calling it a "powerful and diabolical illusion" (125): The brain switches rapidly between tasks rather than performing them simultaneously, each switch incurring a metabolic cost. Multitasking increases cortisol and adrenaline, creates a dopamine-addiction feedback loop, and causes information to be routed to the wrong brain regions.

In organizing social relations, Levitin examines crowdsourcing as collective brain extension, from Amber Alerts to the Defense Advanced Research Projects Agency's (DARPA) Red Balloon Challenge, in which an MIT team located 10 hidden balloons across the United States in under nine hours. He details the fundamental attribution error, the tendency to explain others' behavior through character traits while ignoring situational factors, and presents the Cuban Missile Crisis as a case study in overcoming in-group/out-group bias: Soviet leader Nikita Khrushchev defused the standoff by asking President John F. Kennedy to "try to put yourself in our place" (185), reframing both leaders as members of the same group.

The chapter on time opens with patients who lost the ability to sequence events after damage to the prefrontal cortex. Levitin explains that staying on a single task uses less energy than switching and that the brain automatically segments experience into discrete chunks with beginnings and endings. Sleep is essential for memory consolidation through unitization (binding elements into wholes), assimilation (integrating new information into existing networks), and abstraction (discovering hidden rules). The CDC declared sleep deprivation a public health epidemic, citing its role in disasters from Chernobyl to the Exxon Valdez oil spill. Levitin describes the creative flow state, in which self-criticism and fear circuits deactivate when challenge precisely matches skill.

For medical decision-making, Levitin introduces fourfold tables (two-by-two grids that sort outcomes by disease status and test result to calculate actual probabilities) as tools for Bayesian reasoning (a method of updating probability estimates as new evidence is incorporated). Using a hypothetical disease with a base rate (the background frequency of a condition in the population) of 1 in 10,000 and a test error rate of 2%, he demonstrates that a positive result yields only a 1-in-201 chance of actually having the disease. He applies these tools to prostate cancer: For every 48 surgeries performed, only one life is extended, while patients face an 80% incidence of erectile dysfunction. He critiques alternative medicine, explaining that illusory correlation (falsely perceiving a relationship between unrelated events) and denominator neglect (focusing on apparent successes while ignoring the full comparison set) cause people to believe in ineffective remedies.

In the business world, Levitin traces organizational structure from railroad standardizations of the 1840s through modern hierarchies, contrasting flat structures that encourage collaboration with vertical ones that enable accountability. He identifies locus of control, the degree to which people believe they control their own outcomes, as a key predictor of worker productivity. He cites the Fremont GM plant, where Toyota reopened a failed factory with 90% of the same workforce and made it top-performing within a year by treating employees as "intrinsically motivated partners" rather than "mindless idiots" (320).

Levitin argues that with information now instantly accessible, education must shift from transmitting facts to teaching critical thinking: evaluating sources, distinguishing correlation from causation, understanding sampling bias (distortion caused by nonrepresentative samples), and using estimation techniques such as boundary conditions (upper and lower plausible limits for a quantity) and Fermi problems (estimation questions solved through systematic approximation). He notes that Google used such problems in job interviews, asking candidates to estimate how many piano tuners work in Chicago.

The book closes by reaffirming its central principle: Shift the burden of organizing from the brain to the external world, and accept that any system must include space for items that defy neat categorization. Levitin points to the U.S. Interstate Highway numbering system and the periodic table of elements as elegant examples of externalized information. The junk drawer, both literal and metaphorical, is not a failure of organization but a necessary accommodation for the irreducible messiness of life.
