Set in the world of the Chicago Outfit, an organized crime family governed by strict codes of loyalty, arranged marriages, and male dominance, the novel is narrated by Valentina, a twenty-three-year-old widow whose father Giovanni, an Underboss (a high-ranking officer), has arranged for her to marry Dante Cavallaro, the thirty-five-year-old heir about to become the Outfit's Capo (Boss). The novel opens with a prologue set later in the story: Valentina stands naked in Dante's office, demanding he acknowledge her as his wife and threatening to find a lover if he continues ignoring her. The narrative then shifts back in time.

Valentina's first husband, Antonio, died nine months earlier, reportedly killed by the Russians. Her mother Livia is thrilled about the match with Dante, but Valentina carries a dangerous secret: Her marriage to Antonio was never consummated because Antonio was gay. She married him to help hide his sexuality, a punishable crime in the mafia, and now fears that Dante, known for detecting lies, will uncover the truth. When Dante visits for a formal meeting, he is polite but emotionless, explaining he chose her because she was the only logical option. He still wears his dead wife Carla's wedding ring.

At the Scuderi family Christmas party, Valentina reconnects with her friend Aria, who is married to Luca Vitiello, head of the New York Famiglia (crime family). She also sees her cousin and best friend Bibiana, who is trapped in an abusive marriage to Tommaso, an older member of the Outfit. Dante treats Valentina as an equal on the drive home but does not attempt to kiss her goodnight.

On their wedding day, Dante's father Fiore reminds Valentina bluntly that the marriage is one of convenience and her duty is to produce an heir. That night at Dante's Gold Coast mansion, he shows no interest, turning off the light and leaving her untouched. A painful pattern of distance follows. Gaby, a teenage maid rescued from Russian sex traffickers by the Outfit, gives Valentina a tour but steers her from a closed room where Dante preserves Carla's belongings. The cook, Zita, compares Valentina unfavorably to Carla.

After Bibiana advises Valentina to make the first move, Valentina kisses Dante in bed. He deepens the kiss but stops her when she reaches below his waist. She sneaks into Carla's preserved room and finds photo albums of an adoring younger Dante. He catches her and reacts with fury. After days of cold silence, Valentina confronts him, demanding to know why he visits prostitutes but refuses to touch her. In her anger, she blurts out that he cannot even take his wife's virginity. Dante corners her and presses until she confesses: Antonio was gay and never consummated their marriage. He demands the lover's name, but Valentina refuses, knowing Dante would have to kill the man. Valentina asks for a job, and Dante assigns her to manage one of the Outfit's underground casinos, where she clashes with Raffaele, Aria's cousin, who manages the prostitutes with abusive practices. Dante shares a traumatic detail from his past: His father forced him to kill a man at fourteen, then had soldiers torture Dante repeatedly to build his pain tolerance.

Valentina spots Frank, Antonio's former lover, outside the mansion. When rushing to help Bibiana after another beating from Tommaso, she encounters Frank and arranges a secret meeting. Dante leaves a Valentine's Day gift on his pillow: a white gold necklace with an emerald matching Valentina's eyes. At the meeting, Dante and his guard Enzo, who have been tracking her, appear. Shots ring out from the shadows, wounding Enzo. Valentina grabs Dante's arm and redirects his shot, allowing Frank to escape. Furious and suspecting Russian involvement, Dante withdraws all contact as punishment.

Rocco Scuderi, the Outfit's Consigliere (the Boss's chief advisor), asks Valentina to talk his daughter Gianna into accepting her arranged marriage to Luca Vitiello's brother Matteo. Valentina tries but fails. One night she initiates intimacy with Dante in his office, breaking the stalemate. Afterward in bed, she feels his hand gently cover hers.

Valentina discovers she is pregnant. When she tells Dante, he asks coldly if the child is his. His accusation stems from shame that he and Carla never conceived, something he refused to investigate. Valentina moves into the guest bedroom, vowing not to return until he apologizes. On the anniversary of Carla's death, Dante begins clearing Carla's room for a nursery and asks Valentina to return. Though he has not formally apologized, she agrees.

At a pharmacy, Antonio approaches Valentina, alive and in disguise. He reveals he faked his death by killing a homeless stranger and staging the body, then fled to Mexico with Frank. Now back in Chicago, he has recruited disgruntled Outfit members, including Raffaele, to overthrow Dante. He hands Valentina a vial of poison to slip into Dante's drink. She pretends to agree but immediately tells Dante everything. Though anguished, she drives with Dante hidden in the backseat to the warehouse meeting point. Dante emerges and shoots Antonio, kills one accomplice, and kneecaps Raffaele. Valentina drives home and collapses in the shower. Dante carries her to bed and, when she begs him to make love to her, does so gently and face-to-face for the first time.

The next morning, Valentina slips the poison vial to Bibiana, telling her to put it in Tommaso's breakfast and blame the traitors. Dante deduces what happened but agrees to rule the death as connected to the conspiracy, ensuring Bibiana, now pregnant with Tommaso's child, can remain safely in her home.

At twenty-six weeks, Valentina's water partially breaks. In the hospital, Dante finally opens up, admitting he was wrong to accuse her and confessing his most painful secret: Carla did not die naturally. In her final days of terminal cancer, she begged him to end her suffering, and he injected her with insulin. She died in his arms. He promises to be a better husband and reveals they are having a girl. At thirty-two weeks, doctors perform a C-section. Dante is the first to hold their daughter, whom they name Anna.

In the epilogue, set five weeks after the birth, Valentina and Dante resume their intimate life, emotionally connected and face-to-face. Standing over Anna's crib, Dante promises to protect them until his last breath. Though he never says the words aloud, Valentina sees his love in the way he looks at them. She reflects that despite loneliness, betrayal, and loss, she has finally arrived where she belongs.
