The third installment in the Born in Blood Mafia Chronicles series, the novel is set in a world where powerful Italian-American mafia families govern their territories through arranged marriages, strict codes of honor, and brutal violence. Two organizations dominate this world: the Outfit, Chicago's mafia organization, and the Famiglia, New York's ruling mafia family. The story alternates between the perspectives of Gianna Scuderi, the headstrong second daughter of Outfit consigliere (adviser) Rocco Scuderi, and Matteo Vitiello, the knife-wielding younger brother of Famiglia capo (boss) Luca Vitiello.

The novel opens with a flash-forward: Gianna stares at her bloodied reflection in a bathroom mirror after her father has split her lip. Matteo appears behind her, examines her wound, and offers to kill her father. Gianna is disturbed by how much the offer tempts her, recognizing a capacity for cruelty she has long tried to deny.

The narrative then jumps back in time. Matteo first notices Gianna as a scrawny, outspoken fourteen-year-old, but three years later, when he accompanies Luca to meet Luca's fiancée, Aria Scuderi, before the wedding, Gianna has grown into a beautiful young woman who regards him with open hostility. At the wedding reception, after Matteo rescues Gianna from two men making crude advances, they share a passionate first kiss in a hallway. Gianna's younger sister Liliana (Lily) catches them, but Gianna insists the kiss meant nothing and that she will never marry Matteo.

Matteo, however, is already scheming. He overhears his father, Salvatore Vitiello, and Scuderi discussing plans to marry Gianna to a violent older man. To prevent this, Matteo enlists Luca's help to arrange a match with Gianna instead, framing it as revenge against a girl who insulted him. Salvatore agrees, and within days the deal is done. When her father informs Gianna of the arrangement, she is furious, recognizing that Matteo engineered the marriage right after she rejected him. She calls Aria, devastated, and confesses she sometimes considers suicide as her only remaining act of autonomy, though she knows she could never go through with it.

Several traumatic events deepen Gianna's resolve to flee. After a deadly Russian attack on the Vitiello mansion in the Hamptons, Gianna ventures into the basement and witnesses Matteo and an enforcer named Tito torturing a captured Russian. Lily follows her downstairs, sees the carnage, and has a complete psychological breakdown. Gianna tells them she hopes they all burn in hell and vows to escape before the wedding.

With Aria's help, Gianna devises a plan. She will use one of Luca's forged passports, take cash from his safe, and fly to Amsterdam. During a May visit to New York, the sisters drug and restrain Gianna's bodyguard Sandro with tranquilizer syringes. At JFK, Gianna buys a one-way ticket while Aria drives out of the city as a decoy.

Gianna spends six months on the run across Europe, moving through more than 20 cities under assumed names. She dyes her hair, takes waitressing jobs, and communicates with Aria through a cryptic blog. In Munich, she begins dating Sid, a Canadian musician, but never sleeps with him, privately acknowledging that she still compares every man to Matteo.

Matteo has been hunting her relentlessly. He and two of her father's soldiers, Stan and Carmine, burst into the Munich apartment while Sid is with Gianna on the couch. Matteo hauls Sid off her, his face twisted with fury. Before Matteo can decide Sid's fate, Carmine shoots Sid in the head. Matteo is enraged that Carmine acted without his order, but Stan insists it was Outfit business since Gianna is not yet married into the Famiglia.

In a hotel near the airport, Matteo pins Gianna to the bed and recites her father's permission to do whatever he wants with her. She expects violence. Instead, he releases her and says he does not want to take her by force. He tells her she is going to New York, not Chicago.

Luca flies to Chicago with Aria to support Matteo. At the Scuderi villa, Gianna's father refuses to acknowledge her, calls her "damaged goods," and suggests she be sent to an Outfit sex club. When Gianna protests, he slaps her hard enough to split her lip, the scene from the prologue. Matteo drags her to the bathroom and stitches her wound. Dante Cavallaro, the Outfit boss, declines to intervene as long as the marriage proceeds. A priest is summoned. In a small, joyless ceremony, Gianna says "I do," and they fly to New York that night.

On their wedding night, Matteo discovers Gianna is a virgin. She is furious that he knows, having refused to tell him out of pride. Their early marriage becomes a volatile cycle of sexual intensity and mutual provocation. Matteo has a GPS ankle monitor installed on Gianna to prevent another escape, though it eventually comes off after a series of bets. Beneath the power games, their connection deepens. She reconnects with Lily and her younger brother, Fabi, by phone, defying her father's orders. When Gianna tells Matteo that if he truly cared, he would let her go, he says letting her go is the one thing he will never do.

At the Christmas party hosted by the Bardonis, a powerful Famiglia family whose patriarch has been maneuvering to place his son as Luca's consigliere, Mr. Bardoni follows Gianna to a hallway and forcibly kisses and gropes her. Matteo kills Bardoni with his knife, and they stage the death as a suicide. That night, Gianna catches her own reflection and recognizes what she sees in her eyes: love for Matteo. The realization terrifies her.

The crisis arrives when Russians ambush Matteo and Gianna on a drive home. A high-speed chase ends with their car crashing into a wall. Matteo is unconscious and bleeding heavily, and the car catches fire. Gianna drags him to safety, then begins walking away, telling herself this is the escape she always wanted. After several steps, she stops, whispers that she does not want to love him, and runs back. She calls Luca, who offers her total freedom in exchange for saving Matteo's life. Gianna agrees, though she was already performing CPR before the offer. Matteo stops breathing, but she keeps compressing his chest until he gasps back to life.

Luca takes Matteo to the hospital and orders Gianna to leave. She moves to a hotel but feels hollow. When Aria texts that Matteo has collapsed again, Gianna races to Luca's penthouse. Luca blocks the door, warning that Matteo cannot handle the upheaval. Gianna insists she is not leaving again. She lies beside Matteo, holding his hand until he wakes. She tells him she was already doing CPR before Luca's offer, that she would never have let him die, and that she loves him. Matteo says he has loved her for a long time.

In the epilogue, set in late April, Gianna and Matteo are at the Vitiello mansion beach. After playful roughhousing in the ocean, they join Aria and Luca for a barbecue. Gianna approaches Luca and suggests they try to get along. Luca admits he is starting to change his mind about her. Matteo wraps his arm around Gianna, and she reflects that she can feel free even while bound to him.
