The novel opens with a dark prologue on Fort Lauderdale Beach, where Lucas Davenport, a U.S. Marshal, sits alone at dawn. When two muggers approach, he reveals a handgun, gives them fifty dollars, and warns them off. Something terrible has happened, but the novel withholds the explanation.

The story jumps back five years to July. Barney Hall, a young Coast Guard petty officer stationed in Fort Lauderdale, spots a Mako center console picking up a diver in open ocean with no dive flag in sight while fishing with his wife, Sue, off Pompano Beach. He reports the suspicious activity. The Coast Guard sends a boat to intercept, but a man on the Mako produces a suppressed rifle and kills all three boarding crew members. Sue tends to the dying men while Hall chases the Mako down the Intracoastal Waterway, watching three large men offload black containers into an SUV before torching the boat. Firing wildly, Hall kills one man with a lucky shot, but the SUV escapes. The FBI investigation stalls.

Five months later, Lucas discusses the case with Virgil Flowers, a Minnesota Bureau of Criminal Apprehension agent, at a Minneapolis courthouse. Florida Senator Christopher Colles has asked Lucas to investigate the cold case. Lucas recruits his partner Bob Matees, a fellow marshal, and they join an FBI task force in Fort Lauderdale led by supervisory agent Dale Weaver. The suspected scenario is that heroin was dropped from a freighter onto a reef and recovered by divers, but no leads exist.

Lucas and Bob take a street-level approach. Hall describes the Mako crew as big, sunburnt men who looked like they were from New York or New Jersey, plus a smaller figure who drove the getaway vehicle. By weighing a recovered wet suit, Lucas deduces the diver was a small woman. Through aggressive interrogation of dealers, they learn the operation may be Mafia-connected. Following a chain of informants, they discover that a Black woman was the diver and that hairdresser school graduates from a party boat socialized with the Mako crew. Lucas and Bob track down two of the hairdressers, Alicia Snow and Meredith Duffy, but neither identifies the men. The FBI discovers that a classmate, Patty Pittman, vanished after telling her mother the Mako men might be the Coast Guard killers.

Unknown to Lucas, Snow is secretly dating Jack Cattaneo, one of the Mako crew and a member of Douglas Sansone's Newark-based Mafia organization. Snow warns Cattaneo, who reports to Michael Behan, the group's South Florida boss. Behan devises a plan: use Magnus Elliot, a paroled dealer, to feed Lucas the name of a rival mobster, Don Romano, then ambush the marshals during the raid. Behan's enforcer, Jimmy Parisi, murders Snow and Elliot to eliminate witnesses.

Lucas and Bob set up surveillance across from Romano's warehouse in Florida City. As they move to raid the building, two hired assassins emerge behind them with submachine guns. Weaver spots the gunmen, screams a warning, and opens fire. Both assassins are killed, but Bob is shot in the head and dies instantly. Lucas takes a round absorbed by his vest. Inside the warehouse, agents find cash, computers, and a heroin pipe planted in the dumpster to frame Romano.

Devastated, Lucas realizes they were set up. Romano, baffled by the planted evidence, names Sansone as the real enemy and identifies Cattaneo as a key operative. Duffy tentatively identifies Cattaneo from mug shots. At Bob's funeral in Louisiana, Rae Givens, Bob's usual partner, insists on joining the case.

Lucas orchestrates an elaborate undercover operation, securing FBI Deputy Director Louis Mallard's approval for a working group targeting Sansone's entire network. He recruits Virgil, a certified diver, sending him to Hawaii for 35 days of deep-diving training with former Navy SEALs. In January, Virgil and Rae move into a grimy apartment in Hollywood, Florida, posing as "Willy," an unemployed diver, and "Ally," his girlfriend. They stage encounters with Cattaneo at a Miami Beach deli, and he takes the bait. After background checks and a rigorous dive evaluation, Virgil is hired to recover heroin from the ocean floor.

Virgil makes his first dive from Cattaneo's sailboat, descending roughly 150 feet and using a device that triggers LED lights on the submerged heroin cans. He recovers 11 containers, and the Mafia men pay over $80,000. The heroin travels north to a car wash on Staten Island serving as Sansone's distribution hub. Lucas insists the FBI follow the money rather than seize the drugs, prevailing with backing from Colles and Mallard. Virgil makes two more dives, recovering 16 additional cans. On the boat, Marc Regio, one of Sansone's operatives, reacts involuntarily when Rae asks about the Coast Guard shootings, effectively confirming he was the shooter.

On Staten Island, Lucas and fellow marshal Devlin work to flip one of Sansone's distributors. After one dealer refuses to cooperate, they target Paul Curry, entering his house disguised as gas company workers. Assistant U.S. attorney Ann Wright offers Curry a deal: testify or face life in prison. Curry agrees and makes a documented cash purchase from the car wash, creating a money trail to Sansone. However, Curry reveals that each dealer has a mandatory daily check-in call, and the first arrested dealer has already missed his, meaning Sansone knows something is wrong.

Lucas sends Rae a prearranged warning signal. She is on the boat with Cattaneo, Regio, and Matt Lange, another of Sansone's men, while Virgil is underwater on his fourth dive. Cattaneo receives orders from Behan to kill Virgil and Rae. When Regio raises his gun, Rae draws a concealed pistol and shoots him twice in the heart. She holds Cattaneo and Lange at gunpoint until Virgil surfaces. They return to the marina, where FBI agents arrest Cattaneo and Lange, but Behan escapes his condo.

Events then unfold simultaneously. In New Jersey, an FBI SWAT team raids Sansone's house but finds it empty; Sansone and his wife, Sylvia, have fled. Lucas, Devlin, and FBI agent Kerry race to a locksmith shop in Elizabeth where Sansone collects cash. Sylvia fires at Kerry on the sidewalk but hits her husband in the leg instead. Kerry returns fire, wounding Sylvia. The money man inside is captured holding the documented cash, and Sansone is arrested at the scene. In Miami, Virgil and FBI agent George Hamm race to Opa-locka airport, where Behan's twin-engine plane is already taxiing for takeoff. Hamm drives onto the runway, and Virgil shoots out the nose gear tire, collapsing it. Behan is arrested.

The Coast Guard recovers 700 kilos of heroin. Sansone, Behan, and Cattaneo receive life sentences. Parisi receives life without parole for the murders of Snow, Elliot, and Pittman. Lange receives 15 years for cooperating. Romano walks free. Virgil returns to Minnesota and begins writing a novel. The original diver, known only as Jaquell, lives freely in the Bahamas under a new name, never learning about the convictions.
