Blindsighted is set in the small town of Heartsdale, Georgia. The story centers on Sara Linton, a pediatrician who also serves as the county coroner; her ex-husband Jeffrey Tolliver, the chief of police; and Lena Adams, a detective on Jeffrey's force.

On a Monday afternoon, Sara arrives late for lunch with her younger sister Tessa Linton at the Grant Filling Station diner. In the bathroom, she discovers Sibyl Adams, a blind professor at Grant Institute of Technology, sitting on the toilet with a large cross carved into her abdomen and blood pooling around her. Sara tries to save her, but Sibyl convulses and dies in Sara's arms. Sara calls Jeffrey, who arrives to process the scene. During her preliminary examination, Sara determines that Sibyl was raped, with a deep stab wound at the intersection of the cross cuts from a double-edged blade. The attacker cut a hole in Sibyl's belly and penetrated the wound before raping her vaginally. Sara warns Jeffrey that this is a ritualistic crime and the killer will not stop with one victim.

Jeffrey informs Sibyl's identical twin sister Lena, a detective on his squad. Lena sprints to the morgue and confronts Sara, accusing her of failing to save Sibyl, then attacks Jeffrey before collapsing into sobs. She demands to work the case. That night, Lena drives to the nearby town of Reece to tell her uncle Hank Norton, who raised the twins after their parents died. Hank, who formerly had an addiction, accidentally blinded Sibyl in a drunk-driving incident when the girls were eight, something Lena has never forgiven.

Sara performs the autopsy overnight. Lab results reveal that Sibyl ingested a lethal dose of belladonna, a plant from the deadly nightshade family that causes dilated pupils, elevated body temperature, loss of voice, and selective amnesia. Sara privately tells Lena that Sibyl's intact hymen indicates she was a lesbian. Lena discloses this to Jeffrey, who agrees to keep it confidential. At a briefing, Jeffrey distributes a list of registered sex offenders and pairs Lena with a junior patrolman, deliberately keeping her at the investigation's periphery.

Jeffrey and his senior detective Frank Wallace pursue early leads, but racist undercurrents complicate the investigation. Will Harris, the diner's elderly Black busboy, is targeted with a brick and threatening note thrown through his window. Jeffrey guards Will's house overnight and is hit by buckshot when a side window explodes. Frank resolves the situation quietly, implying he knows who was responsible. Lena, meanwhile, disobeys orders by interviewing Will on her own, finding his alibi airtight. Jeffrey berates her and reassigns Lena as his partner.

On Wednesday, Jeffrey and Lena learn that Julia Matthews, a 23-year-old college student, has been missing for two days. Julia's ex-boyfriend Ryan Gordon demands a lawyer. In a later interview, Lena extracts false confessions that do not match the crime details, confirming Gordon is not the killer.

While Jeffrey recovers from his gunshot wound at the hospital, Sara goes outside and finds Julia's nude body displayed on the hood of her car in a crucifixion pose: arms spread, feet crossed, mouth taped shut with silver duct tape, her driver's license shoved down her throat. Julia's front teeth have been knocked out, her body scrubbed with bleach, and puncture wounds pierce her hands and feet. When Julia's heart stops, Sara cracks her chest and manually massages her heart back to life.

The next day, Lena interviews Julia, who describes being abducted near the college library after bumping into a man who injected her with a needle. She was held in a dark basement with a constant dripping sound. Julia becomes distraught and reveals that her attacker "made love" to her (235). When Lena reacts with shock, Julia grabs Lena's gun and kills herself.

Sara performs Julia's autopsy and finds belladonna berries in the stomach and a small handcuff key in the rectum whose sharp edge tore the attacker's condom, yielding DNA evidence. Tessa confronts Sara and insists she tell Jeffrey about her past: Twelve years ago, while Sara was a resident at Grady Hospital in Atlanta, she was raped. Her attacker sends her a postcard every year.

Sara goes to Jeffrey's house and begs him to hold her. While he sleeps, she leaves the transcript of her rape trial on his bathroom sink. Jeffrey reads how Jack Allen Wright, a hospital janitor, attacked Sara in a bathroom, handcuffed her, taped her mouth, raped her, stabbed her, and poured vinegar into her mouth, saying, "It is finished" (274), quoting Jesus's last words on the cross. Sara became pregnant from the rape, developed an ectopic pregnancy, and had a partial hysterectomy, leaving her unable to have children.

Jeffrey drives to Atlanta to confront Wright, who is now on Depo-Provera, a form of chemical castration. Wright taunts Jeffrey but claims someone else scratched the word "CUNT" on Sara's car at the hospital. Jeffrey breaks Wright's monitoring bracelet and discovers an illegal gun under his mattress. Wright's parole officer, Mary Ann Moon, holds Wright in custody on the resulting parole violations. When Jeffrey calls Sara, she is furious that he went to Atlanta instead of being there for her.

On Saturday, Jeffrey returns to find that Lena has been missing since Thursday night. Frank discovers the word "CUNT" carved inside a kitchen cabinet at Lena's house, matching inscriptions found on Sara's car, Sibyl's classroom chalkboard, and Julia's textbook. Jeffrey has Moon pull employment records from Grady. The 13-year-old patient who died under Sara's care on the day of her rape was Sally Lee McGuire. A search of staff records reveals that Jeremy "Jeb" McGuire, the town pharmacist Sara has been casually dating, completed his pharmacy internship at Grady at the same time.

Lena is captive in Jeb's attic, nailed to the floor and drugged with belladonna. He confesses to killing Sibyl and rapes Lena. At Sara's house, Jeb pours wine laced with vinegar. Sara recognizes him as the killer when she sees a photograph of his dead sister Sally, the same photograph she saw in the Grady waiting room 12 years earlier. Jeb reveals that he raped his 13-year-old sister, who became pregnant and died under Sara's care. He blames Sara and moved to Grant to be near her, leaving victims for Sara to find.

Sara refuses to submit. When Jeb lunges with a knife, she crashes through her bedroom window and sprints to the lake. Jeb chases her in a boat, but Sara swims toward a submerged rock field. The boat strikes the rocks and flips, throwing Jeb into the water. He cannot swim. Despite Sara's attempt to rescue him, his head strikes the boat and he drowns. Jeffrey arrives and pulls Sara from the freezing water.

Jeffrey and Sara search Jeb's house and find Lena nailed to the attic floor behind a false panel. Sara tells Lena that Jeb is dead. Their eyes meet, and Sara understands that Jeb will be hurting Lena every day for the rest of her life.

On Sunday, Jeffrey visits Sara. She tells him she plans to resign as coroner to remove him from her life. Jeffrey declares his love and refuses to give up. While unclogging her kitchen sink, Sara pulls out her old wedding ring, which she told Jeffrey she threw away during the divorce. She holds out her fists and has him pick one. The ring sits in her open palm. Jeffrey pockets it and asks to bring dinner the next night. Sara agrees. The novel ends with the tentative possibility of reconciliation.
