This novel in Karin Slaughter's Will Trent series unfolds across Macon and Atlanta, Georgia, over roughly two weeks, following three protagonists whose lives collide around a violent attack on two police officers and the criminal network behind it.

Detective Lena Adams of the Macon Police Department arrives home after being cleared by Internal Affairs for a raid she led six days earlier on a shooting gallery run by drug dealer Sid Waller. Lena has recently had a miscarriage no one at work knows about, and her marriage to Jared Long, a motorcycle officer with the Macon PD, is fracturing under unspoken grief. That night, she breaks down and embraces Jared. As they kiss, a shotgun blast tears through his back. Lena fights back with a hammer from Jared's tool belt, killing one assailant and disabling another. Before she can finish the second gunman, a third man grabs the hammer. She recognizes him as Will Trent, a special agent with the Georgia Bureau of Investigation (GBI), and understands from his silent signal that he is undercover. When sirens approach, Will disappears, and Lena does not mention his presence to anyone.

In Atlanta, Dr. Sara Linton finishes a shift at Grady Hospital, where she works as a pediatric attending. Sara's first husband, Jeffrey Tolliver, was Jared's late father, a police chief murdered in the line of duty five years earlier. Sara holds Lena responsible for Jeffrey's death. Will, Sara's boyfriend, visits that morning and says his undercover assignment has grown more complicated but shares no details. Minutes later, Sara receives a frantic call from Darnell "Nell" Long, Jared's mother. Jared has been shot, and Lena is involved.

Will reports to his boss, GBI Deputy Director Amanda Wagner. He has been posing as an ex-con named Bill Black at Macon General Hospital, targeting Tony Dell, a petty criminal and hospital maintenance worker connected to a shadowy drug lord called Big Whitey. Dell recruited Will as a lookout for what turned out to be the attack on Lena and Jared. Amanda assigns Will's partner, Faith Mitchell, to investigate. Major Denise Branson from the Macon PD arrives as a liaison but withholds key information, including that she found Will's fingerprints at the crime scene.

A flashback shows Lena's team breaching the foreclosed house and finding three dead men. Waller emerges from a hole behind a basement wall panel and takes a detective hostage. Lena confronts Waller alone. Rather than be taken by a woman, Waller shoots himself. After his suicide, Lena and Branson hear a sound behind the panel. Lena pulls it back. The narrative withholds what she finds.

At Macon General, Sara enters Jared's ICU room and is shaken by how much he resembles Jeffrey. Lena, curled in a corner chair, says she cannot live without Jared. Sara responds: "Good. Now you know how it feels" (108). Will maintains his cover, confronting Dell and fending off Lena's partner, Detective Paul Vickery. Faith arranges for Will to enter Jared's room under the guise of fixing a pipe. Lena refuses to explain the attack but warns Will to stay away if he loves Sara.

Sara visits Lena and Jared's house with Nell and discovers a boot print that matches neither attacker nor any first responder. A FedEx package containing a baby-sized hoodie reveals the couple was preparing a nursery. Faith later shows Sara a medical invoice confirming Lena lost her baby ten days earlier, devastating Sara with the knowledge that Lena had what Sara never could: a child with the man she loved.

That evening, Will dines at the home of Cayla Martin, a hospital nurse. Tony Dell arrives, identifying Cayla as his stepsister. Will notices Cayla's nine-year-old nephew Benji, who is eerily quiet and flinches at contact. Later, Tony drives Will to a strip club back room where men have tortured a bound captive. They force Will to strip to check for a wire and cut him beneath the eye. Tony stabs the captive to death. Will recognizes the victim: Detective Eric Haigh, a member of Lena's raid team.

Past midnight, Lena admits to Sara that she knowingly put Jeffrey's life in danger years ago, then accuses Sara of being attracted to dangerous men and reveals she knows about Sara and Will. Sara nearly strikes Lena but walks away. She then discovers Will outside the hospital in his maintenance uniform, realizing he has been undercover in Macon and that Lena knew while Sara did not: "You let her make a fool of me" (243). Will follows Sara into the basement, where they have an angry, desperate encounter. Afterward, Sara tells him to leave.

Faith learns that attackers also targeted Vickery and DeShawn Franklin, another raid team member, that same night, revealing a coordinated effort to silence Lena's entire team. At a late-night briefing, Branson finally reveals what Lena found behind the wall panel: a young boy, alive but severely traumatized. Branson hid the boy at her girlfriend's farm, believing Big Whitey had a mole in the department who would kill the boy. Branson's obsession with Big Whitey stems from the case of Marie Sorensen, a sixteen-year-old girl Big Whitey abducted, raped, and trafficked until she killed herself. Amanda orders the boy transported to Grady Hospital with Sara as attending physician.

The next morning, Will tells Sara he loves her for the first time. Sara says it back. At the farm, Will coaxes the boy into speaking by telling jokes and promising that bad people get caught. The boy asks about "Benjamin," his brother, who was also held in the basement but was taken away. Sara notes that Benjamin is often shortened to "Benji." Will realizes the quiet boy at Cayla's house is the missing brother.

Will races to Cayla's house and finds Benjamin chained upstairs. He frees the boy and hides him in the attic. Cayla tasers Will from behind. He wakes tied up on the floor. DeShawn Franklin, one of Lena's own team members, sits holding a gun and reveals that Big Whitey is Chief Lonnie Gray, head of the Macon police. Gray raised Franklin and recruited him into the drug operation. Franklin reveals the trigger for the attack: during Lena's ultrasound appointment, Cayla, filling in at the OB-GYN office, overheard Jared joking that Gray might be Big Whitey. Cayla reported this to Gray, who ordered the hit. Lena was never the cause.

Cayla shoots the unconscious Vickery, who arrived after Faith sent him via the police scanner. His Kevlar vest saves him. Tony emerges and kills Franklin but spares Will, believing him a fellow criminal. Tony and Cayla flee. Will contacts Faith using Franklin's phone, which recorded the entire confession. Both boys are safe with Sara. Tony is later captured, and Cayla dies in a crash while fleeing the Atlanta airport.

A flashback confirms the pivotal moment: at their first ultrasound, Jared casually suggests Gray could be Big Whitey. As they leave, the fill-in nurse introduces herself: "I'm Cayla Martin?" (349).

In the final chapter, five days after the resolution, Gray sits in jail after child pornography was found on his computer. Jared is expected to recover fully. Will tells Lena the attack stemmed from Jared's offhand comment, not anything she did. Lena decides not to tell Jared, knowing the guilt would destroy him. She reconciles with Branson and offers to be her partner. Jared hobbles into the station and says he wants to try for another baby. Lena agrees. As she leaves, she drops a postcard addressed to Sara into the mail slot. It reads: "You win" (394).
